Labs & Technicians
All commercial and private dental laboratories operating, doing business, or intending to operate or do business in Kentucky must register with the Kentucky Board of Dentistry.
A dental laboratory shall be considered as operating or doing business in Kentucky if its work product is prepared pursuant to a written authorization originating in Kentucky. A private laboratory performs work exclusively for its own dental office and no other outside work orders. All other labs, which accept outside work orders, are commerical labs.
All commercial laboratories must employ at least one certified dental technician (CDT) who is certified by the National Board of Certification in order to be eligible to register as a commercial dental laboratory.
The information provided below must be submitted for both laboratories and technicians in order for a lab to accept a work order originating from Kentucky.

LAB REGISTRATION INSTRUCTIONS
Submit to the Kentucky Board of Dentistry:
1. Completed lab application sworn before a Notary Public;
2. $50.00 application fee;
3. Photograph of the owner attached to the laboratory registration application;
4. Names of all technicians and CDT's listed on the application in the appropriate space provided;
5. Copy of current CDT certification card for each CDT being registered; and
6. Completed application for each technician or CDT with the $10.00 application fee for each technician or CDT being registered.

Lab Technicians |

Anyone working as a dental laboratory technician (including the laboratory owner) who has two or more years training and/or experience must register with the Kentucky Board of Dentistry.
The application should be carefully completed and sworn to before a Notary Public.
All questions must be completed and a photograph of the registering technician shall be attached to the application.
There is a $10.00 registration fee for each technician or CDT being registered.
If any of the technicians are already registered as dental laboratory technician or a CDT in Kentucky, please write their name and their Kentucky registration number on the application in the space provided.
